De vestingstad en het fort (zogenaamd 'Kleine Fort') werden in de 18e eeuw gebouwd in opdracht van het Oostenrijk-Hongaarse rijk en genoemd naar de keizerin Maria Theresa. Zowel de rivier de Elbe en de Eger stromen nabij het fort. De bouw startte in 1780 en duurde tot 1790; uiteindelijk besloeg het fort een oppervlakte van 3.89 km². Het is ontworpen in de stijl van Sébastien Le Prestre de Vauban en er waren ongeveer 5600 soldaten gestationeerd. Terezín werd tijdens oorlogen niet gebruikt. In de laatste helft van de 19e eeuw diende het fort als een gevangenis. Tijdens de Eerste Wereldoorlog deed het fort dienst als krijgsgevangenenkamp, waar ook de moordenaar van Franz Ferdinand, Gavrilo Princip tot 1916 werd opgesloten. Hij leed aan tuberculose, waar hij uiteindelijk in 1918 in een nabijgelegen ziekenhuis aan bezweek. In de Tweede Wereldoorlog gebruikten de nazi's Theresienstadt als concentratiekamp terwijl het Kleine Fort als gevangenis door de Gestapo in gebruik werd genomen.

Small Fortress (Malá pevnost in Czech, Kleine Festung in German) was part of the fortification on left side of river Ohře. Since 1940, the Gestapo used it as a prison (the largest one in the Protectorate of Bohemia and Moravia). It was separate and unrelated to the Jewish ghetto in the main fortress on the river's right side. Around 32,000 people arrived there and were usually sent to a concentration camp later. 2,600 people were executed, starved, or succumbed to disease there. A possible 1,100 children survived of the 15,000 sent there. Anton Malloth was a notorious prison guard there. After escaping justice for 55 years, he was finally convicted for beating at least 100 prisoners to death, and sentenced to life imprisonment, in 2001.
